Background

Pragmatic trials are increasingly acknowledged as providing evidence from the real world to support clinical decision-making. The term "pragmatic", however, is used inconsistently and its definition and evaluation require clarification. Pragmatic trials should be designed to inform policy and clinical practice decisions, rather than to prove the validity of a clinical or physiological hypothesis. Many RCTs that do not meet the criteria for pragmatism, however, they have characteristics that are in opposition to pragmatism, have been published in journals of varying types and incorrectly labeled pragmatic. This can result in misleading claims of pragmaticity, and the use of the term should be standardized. The creation of a PRECIS-2 tool that provides an objective, standardized evaluation of the pragmatic characteristics is a first step.

Methods

In a pragmatic trial it is the intention to inform policy or clinical decisions by demonstrating how the intervention can be incorporated into real-world routine care. This is distinct from explanation trials, which test hypotheses about the causal-effect relationship in idealized situations. Consequently, pragmatic trials may have less internal validity than explanatory trials and may be more susceptible to bias in their design, conduct and analysis. Despite these limitations, pragmatic trials can be a valuable source of information for decision-making in healthcare.

The PRECIS-2 tool assesses the level of pragmatism that is present in an RCT by assessing it across 9 domains, ranging from 1 (very explicit) to 5 (very pragmatic). In this study the areas of recruitment, organization as well as flexibility in delivery flexible adherence, and follow-up scored high. Conclusions

In recent years, pragmatic trials are gaining popularity in research as the importance of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that compare real-world care alternatives rather than experimental treatments under development. They include patient populations that more closely mirror the patients who receive routine medical care, they utilize comparisons that are commonplace in practice (e.g. existing medications), and they depend on participants' self-reports of outcomes. This method is able to overcome the limitations of observational research, like the biases that come with the reliance on volunteers, as well as the insufficient availability and codes that vary in national registers.

Other advantages of pragmatic trials include the ability to utilize existing data sources, as well as a higher chance of detecting meaningful changes than traditional trials. However, they may still have limitations that undermine their reliability and generalizability. For example, participation rates in some trials may be lower than anticipated due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g., industry trials).
